摘要:
The prevalence of, and related factors to, stress urinary incontinence (SUI) among perimenopausal Chinese women and its impact on daily life among those women with sexual desire problem in Hubei province were investigated. In this study, 1519 perimenopausal women aged 40 to 65 years were selected from three urban communities in the Wuhan area, and two impoverished, mountainous communities in Hubei province, and followed from April to October 2014. Detailed information about demographic characteristics, menstruation, pregnancy, sexual life and chronic diseases was collected. A cross-sectional survey was carried out following information collection by Chi-square test and multiple logistic regression analysis. Univariate and multivariate logistic regression analysis demonstrated that the potential factors associated with developing SUI were old age (OR=3.4, 95% CI: 1.92–6.04), vaginal delivery (OR=0.623, 95% CI: 0.45–0.87), low income (OR=0.063, 95% CI: 0.40–0.92), atrophic vaginitis (OR=1.4, 95% CI: 1.03–1.80), pelvic organ prolapse (OR=2.81, 95% CI: 1.36–5.80), chronic pelvic pain (OR=2.17, 95% CI: 1.90–4.03), constipation (OR=1.44, 95% CI: 1.07–1.93) and incontinence of feces (OR=3.32, 95% CI: 2.03–5.43). Moreover, the ratio of SUI (33.2%) was higher than the ratio of urgency urinary incontinence (24.1%) or the ratio of mixed urinary incontinence (17.4%), and SUI had a greater impact on daily life among women with decreased sexual desire. In conclusion, SUI is a common disorder affecting over one third of the women surveyed, and has a severe impact on the daily life of perimenopausal women with declined sexual desire. Age, mode of delivery, and monthly income are major risk factors involved in the development of SUI.

Cadmium (Cd) is a well-recognized, hazardous toxic heavy metal, and the adverse effects of high-level Cd exposure on human health have been well documented. However, little is known about the health effects of low-level environmental Cd exposure on pregnant women. The objective of this study was to assess urinary metabolic alterations in pregnant women exposed to environmental Cd, and to identify informative biomarkers. Urine samples from 246 pregnant women in the first trimester of pregnancy were collected, and urinary Cd concentrations were quantified using inductively coupled plasma mass spectrometry (ICP-MS). Urinary metabolomics was analyzed by ultra-performance liquid chromatography coupled to quadrupole time-of-flight mass spectrometry (UPLC-QTOF-MS). Cd-related metabolic biomarkers were examined by comparing the samples of the first and third tertiles of Cd exposure classifications, using a partial least-squares discriminant (PLS-DA) model. Five putative biomarkers were identified, including L-cystine, L-tyrosine, dityrosine, histamine, and uric acid, all of which were related to oxidative stress and nephrotoxic effects induced by Cd exposure. The results show that low-level environmental Cd exposure could induce metabolite profile alterations in pregnant women, which might be associated with adverse health effects. Our findings provide new insights into the early molecular events following Cd exposure, and may be valuable for the health risk assessment of Cd exposure during pregnancy. © 2019

Pregnant women, a vulnerable population, can be exposed to a variety of environmental metals that may adversely affect their health at elevated concentrations. Moreover, environmental exposure and risk disparities are associated with several factors such as sociodemographic characteristics, pregnancy complications, and nutritional supplementation indicators. Our aim was to investigate whether and how the urinary metal concentrations vary according to these factors in a large pregnancy and birth cohort. Urinary levels of seven metals including manganese, zinc, selenium, arsenic, cadmium, thallium, and lead were assessed in 7359 participants across Wuhan City in China. In all, 7359 urine samples from the third trimester were analyzed and 15 sociodemographic characteristics, pregnancy complications, and nutritional supplementation indicators were assessed: maternal age, household income, multivitamin supplements during pregnancy, etc. Concentrations of Zn, As, and Cd were detected in all urine samples; Mn, Se, Tl, and Pb were detected in 95.3%, 99.8%, 99.8%, and 99.9% urine samples, respectively. The geometric mean (GM) of creatinine-adjusted urinary Mn, Zn, Se, As, Cd, Tl, and Pb concentrations were 1.42, 505.44, 18.24, 30.49, 0.64, 0.55, and 3.69 mug/g. Factors that associated with environmental metals concentration were as follows: (1) Urinary Zn and Cd concentrations increased with maternal age. (2) Pregnant women with higher education had lower Cd concentrations. (3) Pregnant women with folic acid supplementation had lower Mn and with multivitamin supplementation had higher Se and lower Tl. Our results demonstrated that the maternal age, education level, folic acid supplementation, and multivitamin supplementation were significantly associated with environmental metal concentrations. Furthermore, studies are recommended to explore the influence of diet on biological metals concentrations in more detail.

2,4-Dichlorophenoxyacetic acid (2,4-D) is a chlorophenoxy herbicide used worldwide. We describe a high-performance liquid chromatography (HPLC) method with UV detection for the determination of 2,4-D in female and male rat serum. This allows to observe the change of serum 2,4-D concentration in rats with time and its pharmacokinetics characteristics with a simple, rapid, optimized and validated method. The serum samples are pretreated and introduced into the HPLC system. The analytes are separated in a XDB-C18 column with a mobile phase of acetonitrile (solvent A) and 0.02 M ammonium acetate (containing 0.1% formic acid) (solvent B) using a gradient elution at a flow rate of 1.0 mL/min. The wavelength for UV detection was set at 230 nm. Calibration curve for 2,4-D was constructed over a range of 0.1-400 mg/L. The method was successfully applied to study the pharmacokinetics of 2,4-D in rats in this study. After oral administration of 300 mg/kg and 60 mg/kg 2,4-D, the mean Cmax values were 601.9 and 218.4 mg/L, the AUC0-->infinity values were 23,722 and 4,127 mgxh/L and the clearance (Cl) were 1.10 and 0.02 L/(hxkg), respectively. The developed method was found to be specific, precise, reproducible and rapid.

目前传统面对面的课堂教学模式(traditional learning,T-Learning)已经无法满足医学实践教学发展的需要.如何帮助学生汲取专业知识,提升专业技能,把握学科动态,加强信息素养,培养创新能力,已成为我国当前医学教育迫切需要解决的问题.混合式学习(blended learning,B- Learning)是继电子化教学(electronic learning,E-Learning)模式发展演变而来的一种学习方法和教学理念.20 世纪 90 年代,随着信息技术的高速发展,E-Learning作为一种提倡以学生为主体、自定义步调进行个性化学习的教学模式,受到教育界的高度重视.然而,随着研究的不断深入,E-Learning 教学模式的不足也逐渐凸显出来,主要表现为较高的辍学率,学习的易干扰性,人际交互的缺乏,知识掌握的碎片化,缺少定制课程等一系列问题.2000 年美国"教育技术白皮书"指出:E-Learning 主要通过因特网进行学习和教学,不能取代课堂教学[1].学者们对 E-Learning教学进行反思后提出了"B- Learning"教学理念,即传统教学(T-Learning)和电子化教学(E-Learning)的结合.田世生指出,T-Learning和E-Learning并不是简单意义的结合,"混合式"的内涵,应该是各种学习方法、学习媒体、学习内容、学习模式以及学习支持服务和学习环境的混合[2].混合学习所要做的工作是在适当的时间,为适当的人,以适当的传递媒体,通过适当的学习方式,提供适当的学习内容,最终达到最优的教与学效果,这样既发挥教师的引导、启发、监控教学过程的主导作用,又充分体现学生作为学习主体的主动性、积极性和创造性[3].

摘要:
采用儿童气质评估系统对504名618个月的婴幼儿气质进行测评,分析婴幼儿的气质特点和分布类型。结果表明:504名婴幼儿的气质类型以中间型和易养型为主(分别占48. 4%和43. 5%),而启动缓慢型和难养型的比例分别为5. 4%和2. 6%;部分气质维度在不同年龄和性别间存在差异。婴儿组和幼儿组在气质类型分布上有统计学差异(P <0. 05),婴儿组儿童缓慢型和难养型的比例要高于幼儿组儿童,反之易养型的比例低于幼儿组儿童。男童在活动水平维度上得分高于女童(P <0. 05)。

摘要:
Thallium (Tl) is a well-recognized hazardous toxic heavy metal that has been reported to have embryotoxicity and fetotoxicity. However, little is known about its association with preterm birth (PTB) in humans. We aimed to evaluate the predictors of Tl exposure and assessed its relation with PTB. The study population included 7173 mother-infant pairs from a birth cohort in Wuhan, China. Predictors of Tl concentrations were explored using linear regression analyses, and associations of Tl exposure with risk of PTB or gestational age at birth were estimated using logistic regression or generalized linear models. The geometric mean and median values of urinary Tl concentrations were 0.28 μg/L (0.55 μg/g creatinine) and 0.29 μg/L (0.53 μg/g creatinine). We found that maternal urinary Tl concentrations varied by gestational weight gain, educational attainment, multivitamin and iron supplementations. Women with Tl concentrations higher than 0.80 μg/g creatinine were at higher risk of giving birth prematurely versus those with Tl concentrations lower than 0.36 μg/g creatinine [adjusted odds ratio (95% confidence interval (CI)): 1.55 (1.05, 2.27)], and the association was more pronounced in PTB with premature rupture of membranes (PROM) rather than in PTB without PROM. About 3-fold increase in creatinine-corrected Tl concentrations were associated with 0.99-day decrease in gestational length (95% CI: −1.36, −0.63). This is the first report on the associations between maternal Tl exposure and the risk of PTB. © 2017 Elsevier Ltd

旨在回顾各种不同干预方法对脑瘫患儿生命质量的影响,本文从身体训练、药物治疗和/或手术治疗、行为训练以及综合康复训练四个方面对相关研究进行综述。本文研究结果表明综合运用各种治疗手段替代传统单一的治疗方法是当今脑瘫患儿临床干预的发展趋势。然而,从生命质量视角判定现有干预方法有效性尚未得到一致结果,研发有效的、统一的测评工具,开展严谨的临床实践研究是未来该领域的研究方向,深入研究脑瘫患儿生命质量可指导临床干预措施的选择。

摘要:
腰椎间盘突出症(LDH)是由于各种原因导致腰椎间盘纤维环破裂,髓核组织从破裂口突出,压迫神经根及马尾神经而产生的一系列以疼痛和神经功能障碍为主的症候群[1].其临床发病率高,容易复发,给患者身心带来痛苦和不便,且耗费了大量的医疗资源[2-3].腰椎间盘突出症的治疗手段较多,针刺、艾灸等传统康复治疗手段效果明显,且患者易于接受[4].本研究选取了腰椎间盘突出症中较为常见的寒湿型腰痛作为研究对象,采用改良艾灸器结合针刺的康复干预方法,疗效满意,现报告如下.

摘要:
尽管已有研究初步显示社交媒体干预在重性精神疾病患者干预治疗方面具有潜在优势,然而缺少对其有效性的探索。本文旨在探究重性精神疾病患者社交媒体干预的有效性,从网络同伴支持干预、网络家庭干预、网络心理健康教育干预以及移动设备干预四个方面对现有相关研究进行分析。结果表明由于现有研究的局限性,使得关于社交媒体干预的有效性尚无法得出确定的结果。需要未来进一步通过高质量的研究,全面评价社交媒体干预在重性精神疾病患者运用的有效性和潜在风险。

摘要:
目的探讨儿童保健护理服务需求的属性,找出儿童看护人群期望的服务需求属性,为其优质护理提供实施方案。方法基于Kano模式自行设计儿童保健护理服务需求调查问卷,对300名儿童看护人群进行问卷调查。结果 30项护理服务需求中,1项为必备属性,12项为期望属性,17项为魅力属性。象限图处于优势区有4项服务,处在改进区有11项服务。结论护理管理者应该根据Kano分析的结果及结合象限图分层次改进护理服务,提高护理品质。

摘要:
目的:了解护理硕士专业学位研究生临床实践能力培养效果.方法:采用整群抽样对某校2015级45名护理硕士专业学位研究生进行问卷调查.结果:无论是否有临床护理工作经历,经过研究生临床实践培养,学生的理论知识 、护理专科操作技能和护理科研能力均明显提高(P<0.05);无临床护理工作经历的研究生护理基本操作技能和交流沟通与协作能力有不同程度提高(P<0.05);研究生的批判性思维能力、临床决策力、临床护理管理能力和教学能力经过临床实践培养提高均不明显(P>0.05).结论:建议护理硕士专业学位研究生在临床实践开始前首先评估学生的临床实践能力,有针对性的安排实践内容,制定个性化的临床实践培养方案.

Arsenic exposure is considered a major environmental threat to human health. It is already known that high-level arsenic exposure has adverse effects on human health. Since the pregnant women are known to be more susceptible to some chemical exposures than ordinary people, the understanding regarding the health effects of low-level arsenic exposure on pregnant women is critical and remains unclear. The aim of this study is to investigate the urinary metabolic changes of pregnant women exposed to low-dose arsenic, and to identify biomarkers from metabolomics analysis. Urine samples of 246 pregnant women were collected in the first trimester of pregnancy and were divided into three groups based on the tertile distribution of urinary arsenic concentrations which were determined using inductively coupled plasma mass spectrometry (ICP-MS). Changes in the metabolite profile were measured using ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry (UPLC/Q-TOF MS). Arsenic- related metabolic biomarkers were investigated by comparing the samples of the first and third tertiles of arsenic exposure classifications using a partial least-squares discriminant model (PLS-DA). Nine urine potential biomarkers were putatively identified, including LysoPC (14:0), glutathione, 18-carboxy-dinor-LTE4, 20-COOH-LTE4, cystathionine ketimin, 1-(beta-D-ribofuranosyl)-1,4-dihydronicotinamide, thiocysteine, p-cresol glucuronide and vanillactic acid. The obtained results showed that environmental arsenic exposure, even at low levels, could cause metabolite alterations in pregnant women which might be associated with adverse health outcomes. This is the first report on metabolic changes in pregnant women for arsenic exposure. The findings may be valuable for the arsenic risk assessment for pregnant women. © 2016 Elsevier B.V.
